On Monday, May 22, 2023 (5/22/2023), we are changing our phone menu options.

Individual Providers (IP) and Clients will need their Person ID number to directly reach their assigned Service
Coordinator.

You can find your Person ID in the DirectMyCare web portal (https://directmycare.com/#/login).

• After logging in, click on your name in the upper right-hand corner of the screen.

• Your Person ID is located in the “Basic Information” section.

information system is always available. Call 866-214-9899 and press option #1.
